fix(converter): 修复 nginx 配置解析空字符串和 upstream URL
- 解析器正确处理空引号字符串 ""，使用标记区分空字符串和特殊字符
- upstream server URL 自动添加 http:// 前缀以满足 lolly 配置验证
- proxy_set_header 正确处理空字符串值（如 Connection ""）

- 为导出的函数添加注释 (revive)
